Biomechanical Effects of Cement Augmentation in the Treatment of Intertrochanteric Fractures with Compression Hip Screw Systems in Relation to Osteoporosis of Varying Degrees

چکیده
Introduction: Screw cut-out and non-union have been cited as major complications with hip screw systems for the treatment of intertrochanteric femoral fractures. Recently, cement augmentation of hip screw system has been introduced to provide better purchase of the screw. However, its effectiveness is not entirely clear when the various degrees of osteoporosis has progressed within the femoral head region. This study investigates the biomechanical efficacy of cement augmentation technique for varying degrees of osteoporotic femur. For this purpose, the stress distributions within the femur and the surgical construct was assessed. The relative movement at the interfacial junctions were predicted to assess its fixation strength that is critical for post-op bony healing.

Comparison of Intertrochanteric Fracture Fixation with Dynamic Hip Screw and Bipolar Hemiarthroplasty Techniques
Background: At present, there is no consensus on an appropriate treatment modality for intertrochanteric fractures in the elderly with background diseases. The aim of the present study was to compare treatment outcomes of intertrochanteric fractures reduced with dynamic hip screws (DHS) and bipolar hemiarthroplasty in elderly patients with background medical conditions. Methods: In this randomi...
متن کاملType II Intertrochanteric Fractures: Proximal Femoral Nailing (PFN) Versus Dynamic Hip Screw(DHS)
Background: Intertrochanteric fracture is one of the most common fractures of the hip especially in the elderly with osteoporotic bones, usually due to low-energy trauma like simple falls. Dynamic Hip Screw (DHS) is still considered the gold standard for treating intertrochanteric fractures by many. Not many studies compare the DHS with Proximal femoral nail (PFN), in Type II intertrochanteri...

Cut out or extrusion of the lag screw from the superior aspect of head and neck of the femur is one of the most common and devastating complications of the surgery of the intertrochanteric fractures with DHS. The exact cause of this complications is unknown, but it seems to be related to osteopenia, inappropriate position of lag screw inside head of the femur and inability of DHS to slide insid...
متن کاملHow to Treat the Complex Unstable Intertrochanteric Fractures in Elderly Patients? DHS or Arthroplasty
Background: Due to difficulty in obtaining anatomical reduction, management of the unstable intertrochanteric fractures in elderly osteoporotic patients is challenging. The purpose of this study is to compare the results of hip arthroplasty (total, hemi, or bipolar) with DHS in the elderly patients with unstable intertrochanteric fractures. Methods: We prospectively studied and followed-up ...

With rise in Osteoporosis and geriatric population the risk of getting intertrochanteric fracture is on rise. Rigid fixation with early mobilization of patients is the standa...
